Jen Horonjeff, PhD, is a patient advocate and the Founder & CEO of Savvy Cooperative. Jen was named one of the 50 Most Daring Entrepreneurs by Entrepreneur Magazine for her innovative approach to empowering patients to use their health experiences to advance research and product development while being fairly valued for their efforts. Driven by purpose, Jen is deeply committed to giving patients a voice as she grew up with multiple autoimmune diseases and survived a brain tumor as an adult. She is also a patient-centered outcomes researcher, a human factors engineer, a former FDA advisor, and serves as an advisor and board member for numerous organizations. Utilizing her unique perspective, Jen strives to give patients a platform to have their experiences be heard, included, and equitably valued—because we need to #AskPatients as we co-design the future of healthcare.

Upon recognizing significant unmet needs and challenges in patients’ experiences throughout their cancer diagnosis, Dr. Grace Cordovano, PhD, BCPA, founded Enlightening Results in 2010. Dr. Cordovano is dedicated to fostering personalized patient advocacy services, specializing in oncology, rare, and catastrophic cases. She has been likened to the “House” of patient navigation, internationally recognized for “navigating the unnavigable.”  She strategically guides patients and their carepartners through survivorship or end-of-life care planning with empathy, ensuring individuals are armed with the most pertinent, medically credible, easy-to-understand information, tools, and technology to make informed decisions about their care. Dr. Cordovano is a champion for palliative care, digital health, clinical trials, and harnessing the power of AI in navigating life with a cancer diagnosis. She amplifies that patient care strategies must give a competitive advantage to the patient, not their diagnosis.

marker TOPIC: Driving the Future: The Symbiotic Evolution of Digital Health and the e-Patient

A practicing physician with training and experience in clinical informatics, Dr. Sands has worked in a variety of capacities in the healthcare IT industry for over the past 30 years and is a co-founder and the chief advocacy officer of the Society for Participatory Medicine. His digital technology career began at Beth Israel Deaconess Medical Center, where he trained in medical informatics and developed and implemented innovative systems to improve clinical care delivery and patient engagement, including clinical decision support systems, an EHR, and one of the nation’s first patient portals. This was followed by leadership positions including Cisco, Zix Corporation, and others.

Dr. Sands has earned degrees from Brown University, Ohio State University, Harvard School of Public Health, and trained at Boston City Hospital and Boston’s Beth Israel Hospital.

Dr. Sands holds an academic appointment at Harvard Medical School and maintains a primary care practice at Beth Israel Deaconess Medical Center. In addition to practicing, Dr. Sands is currently the chief medical officer of WKD.SMRT.
